O1 Visa Requirements Checklist

Must be machine-readable and undamaged. Provide clear copies of all pages, including any previous passports if relevant. Cross-reference with visa application form.

Include visa pages, I-94 records, and prior petition approvals. Required if applicant has previously held US status. Ensure legibility and completeness.

Police clearance certificates from all countries where applicant has lived for 6 months or more since age 16. Required for certain countries and may be requested by consular officer.

Ensure all fields are accurately completed for O1 category (A or B). Must be signed by the US petitioner and dated within 60 days of submission. Include the O Supplement with detailed information about the extraordinary ability and proposed work.

Must be on official letterhead, signed by authorized representative. Should address the applicant's extraordinary ability and proposed duties. May be waived if no peer group exists—attach explanation. Required for most O1 petitions.

List all engagements with dates, locations, and nature of activities. Should cover the entire requested validity period. Attach supporting invitations or agreements as available.

Each letter should be on official letterhead, signed in ink, and include the expert's credentials. Letters must describe applicant's achievements, relationship, and specific examples of extraordinary ability. Originals preferred; certified copies accepted with explanation.

Include original award certificates and published announcements. Where awards are not available, provide alternative evidence from other checklist categories. Notarized translations required for non-English documents.

Submit full copies of articles, cover pages, and publication details. Highlight applicant's name and relevance. For online materials, include URLs and access dates.

Membership certificates must show eligibility criteria. Provide association bylaws or membership standards. Attach evidence of active participation if applicable.

Include box office receipts, record sales, or other commercial success metrics. Provide evidence of critical or commercial success in the performing arts. Specific to O1-B category for performing artists.

Include patents, inventions, research papers, creative works, or other original contributions. Provide evidence of impact and recognition in the field. Critical for demonstrating extraordinary ability.

Document leadership positions, board memberships, or critical roles in prestigious organizations. Provide organizational charts and evidence of responsibilities.

All documents not in English must be accompanied by certified translations. Translations must include certification statement, translator's credentials, and signature.

Prepare for consular interview with knowledge of O1 requirements, evidence of extraordinary ability, and be ready to answer detailed questions about the petition and proposed work.

Contract must detail terms, duties, wages, and duration. If oral, provide detailed summary from both employer and employee, signed and dated. Cross-reference with itinerary and job description.

Job description must specify roles, required qualifications, and how the position requires extraordinary ability. Should be on company letterhead and signed by HR or relevant manager.

Include W-2s, pay stubs, contracts, and industry salary surveys. Documents should cover the last 3 years. Highlight above-average compensation with supporting data.

Include Articles of Incorporation, business licenses, tax ID, and evidence of business legitimacy. Demonstrates the petitioner's ability to support the O1 beneficiary.

Medical examination by approved panel physician if required by specific consulate. Not required for all O1 applications but may be requested for certain countries or circumstances.
