Employment Application Form

HR Recruiting Resource

Standardize recruiting intake with an employment application form for HR and hiring managers. Collect applicant contact details, position sought, work history, education, eligibility to work, and a signed certification that the information is accurate. Distinct from the existing employee-application-form page: this slug is employment-application-form. Follow FCRA, EEO, and state limits on questions (salary history, criminal history, and age). Not legal advice; confirm current application-law rules for each hiring location.

Frequently Asked Questions

How is this different from employee-application-form?
The library already has employee-application-form. This page uses the employment-application-form slug so both URLs can exist. Use one consistently in your ATS; do not overwrite the other JSON file.
What questions should we avoid?
Many jurisdictions restrict salary-history questions, premature criminal-history questions, and items that reveal protected characteristics. Age, disability, and pregnancy questions are high risk. Have counsel review the form for each state you hire in.
Should background-check consent live on the application?
Usually no. FCRA disclosure and authorization are often standalone. Collect the application first; use the background-check authorization form when you are ready to order a report.
Is this an offer or a contract?
No. The application is a candidate submission. Employment terms belong in the offer letter. State that providing an application does not create a job or a contract.
