Employee Handbook Acknowledgment Form

HR Policy and Onboarding Resource

Record that an employee received the current handbook with an acknowledgment form for HR and people operations. Collect the employee’s name, handbook version or effective date, how the handbook was delivered, and a signature stating the employee had a chance to read it and understands employment remains at-will where that is your policy. Acknowledgments help show policies were distributed; they do not replace training or legal review of the handbook itself. Not legal advice.

Frequently Asked Questions

Why collect a handbook acknowledgment?
It documents that the employee received the current policies and had an opportunity to read them. That record is useful if a later dispute turns on whether a policy was communicated. It is not a substitute for a well-drafted handbook.
Should the form say the handbook is not a contract?
Many U.S. employers include language that the handbook does not create a contract and that employment is at-will if that is the intended relationship. Counsel should confirm wording for your states. Do not rely on this form alone to preserve at-will status.
Do we need a new signature after every edit?
Collect a new acknowledgment after material updates (conduct, leave, harassment, pay, or at-will language). Minor typo fixes usually do not require a new form. Keep the version the employee signed.
Can this be signed electronically?
Yes if your e-sign process identifies the signer and retains the signed copy with the version hash or date. Offer a paper option if an employee cannot access the portal.
