Additional Insured Request Form

Risk Management and Insurance Coordinator Resource

Request additional-insured status with a form for risk managers, contractors, and insurance coordinators. Collect the named insured, policy numbers, the additional-insured party’s legal name and address, the contract or job that requires the endorsement, requested wording (ongoing vs completed operations), and how the certificate should be delivered. Additional-insured grants are endorsement- and policy-specific; a certificate alone does not change coverage. Not insurance or legal advice; confirm current endorsements with the broker and carrier.

Frequently Asked Questions

Does a certificate of insurance make someone an additional insured?
No. A COI is evidence. Additional-insured status comes from the policy endorsement and its wording. Ask for the endorsement form number (for example CG 20 10) when the contract specifies it.
What wording should we request?
Copy the contract: ongoing operations, completed operations, primary and noncontributory, and waiver of subrogation if required. Do not invent broader wording than the carrier will issue.
Who submits this form?
Usually the named insured’s broker or certificate clerk, using details the requesting party provides. Incomplete legal names and missing job descriptions are the usual delay.
Is this insurance advice?
No. Coverage depends on the policy and endorsements in force. Confirm with the licensed broker and the written policy. Certificates can contain errors.
